Telehealth Consent

At Vieora Health PLLC (“Vieora”), your care is delivered through telehealth. This consent explains what that means, the benefits, the risks, and your choices, so you can make an informed decision. Please read it carefully and ask us anything before you agree.
What telehealth is
Telehealth, also called telemedicine, is the use of secure video, audio, messaging, and related technology to provide health care when you and your provider are in different locations. At Vieora, your visits with Wahiba Kartaoui, MSN, FNP-C, and any care team member take place by live video, with secure messaging and document sharing between visits.
The benefits
Telehealth lets you receive thoughtful, specialized care for perimenopause, menopause, and metabolic health from wherever you are, without travel or waiting rooms. It can make care more convenient, more private, and easier to fit into your life.
The risks and limitations
Telehealth is not the same as an in-person visit, and you should understand its limits:
- A provider cannot physically examine you, which means some conditions cannot be fully assessed remotely and may require in-person care, laboratory testing, imaging, or referral.
- Technology can fail. Connections may drop, audio or video may be poor, and a visit may need to be rescheduled or completed by phone.
- Although we use secure, encrypted platforms and reasonable safeguards, no technology is completely private or secure, and there is a small risk that protected health information could be accessed by others.
- In rare cases, lack of access to your complete records or to an in-person exam could affect clinical decisions.
Telehealth is not for emergencies
Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, including chest pain, difficulty breathing, severe bleeding, thoughts of harming yourself, or any life-threatening symptom, call 911 or go to the nearest emergency room. Do not use Vieora messaging or scheduling for urgent or emergency needs.
Your location and our licensure
You must be physically located in a state where your Vieora provider is licensed at the time of each visit. Vieora currently serves patients in North Carolina, with additional states added over time. If you travel, tell us before your visit, because we may not be able to provide care while you are in a state where we are not licensed.
Privacy and confidentiality
Your telehealth visits and records are protected under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. We use secure platforms and reasonable administrative, technical, and physical safeguards. Please join your visits from a private place, and do not record a visit without our written permission. We will not record your visit without your permission.
Prescriptions
Any prescription is at the clinical discretion of your provider, based on a good-faith evaluation, and is never guaranteed. Certain medications, including controlled substances, carry additional legal requirements and limitations on telehealth prescribing, and may require in-person evaluation or may not be available through Vieora.
Your alternatives
You always have the right to seek in-person care instead of telehealth. We are glad to discuss whether telehealth is appropriate for your needs and to help you understand your options.
Your right to withdraw
Your participation is voluntary. You may withdraw this consent and stop telehealth care at any time, without affecting your right to future care, by notifying us in writing. Withdrawing consent does not change any obligations already incurred.
Financial
Vieora is a direct-pay membership practice. We do not bill insurance. Membership terms, fees, and refund policies are described in your membership agreement.
Your consent
By accepting, you confirm that you have read and understood this consent and have had the chance to ask questions; you understand the benefits, risks, and limitations of telehealth; you understand telehealth is not for emergencies; and you consent to receive care from Vieora Health through telehealth.
